Job Summary Join Acentra Health as a PASRR Evaluator in Yakima, Washington. In this full‑time hybrid role, you will conduct PASRR Level II Pre‑Admission Screening and Resident Review assessments to determine appropriate placements and next levels of care for individuals with complex needs. The position involves traveling to nursing homes, hospitals, and community‑based settings, with remote work from a home office available. Travel expenses, including mileage at the federal rate and parking, are reimbursed.

Job Responsibilities

Conduct PASRR Level II assessments by reviewing records, gathering collateral information, and evaluating the individual’s physical, behavioral, and psychological functioning.

Perform Preadmission Screening and Resident Review assessments, develop informed level‑of‑care recommendations, and determine the most appropriate next level of care.

Review medical records and interact with healthcare professionals, patients, families, and support systems to achieve accurate screenings, evaluations, and determinations.

Provide well‑informed level‑of‑care recommendations based on clinical evaluations, ensuring adherence to established workflows and completing re‑reviews when necessary.

Collaborate effectively with healthcare professionals, patients, families, and support networks while meeting turnaround time requirements.

Leverage technology, computer equipment, web‑based software, and proprietary platforms to manage and track all assessment activities.

Maintain compliance with regulatory requirements and best‑practice standards, including HIPAA Privacy and Security Rules.

Required Qualifications

Active, unrestricted license in Washington in a healthcare or related field (e.g., LP, LPCC, LMHC, LMFT, LCSW, LMHP, RN with a master’s in psychiatric nursing or related field).

Graduated from an accredited college with a relevant degree and at least 3 years of experience assessing individuals with mental health or psychiatric disorders, including complex differential diagnoses.

Ability to assess and analyze psychological, emotional, behavioral, and physical functioning for accurate clinical evaluations.

Experience in medical record abstraction and clinical data review.

Familiarity with mental health, intellectual and developmental disabilities, geriatrics, and other conditions requiring long‑term care services.

Strong communication skills and empathy in verbal, written, and electronic interactions.

Preferred Qualifications

Basic knowledge of PASRR requirements and compliance with state and federal regulations.

Knowledge of DSM psychiatric disorders and Level‑of‑Care standards, with experience diagnosing conditions using Washington‑state specific criteria.

Detail‑oriented, excellent organizational skills, and the ability to prioritize tasks efficiently.

Previous experience working with the geriatric population.

Proficiency in Microsoft Office Suite (Word, Excel, Outlook, Teams).

Compensation Annual base salary: $85,280.00 – $91,520.00. Pay placement within this range depends on experience and skill level.
