Pre-Transition Specialist – BCFS Health & Human Services Join us to make a difference for youth transitioning out of foster care. We’re seeking a dedicated Pre‑Transition Specialist to support these young adults in reaching their goals.

Education & Experience

Associate’s degree or 60 college hours in social services or a related field.

Minimum of 1 year of full‑time paid experience working with youth/young adults in housing, education, or employment programs.

Job Summary The Pre‑Transition Specialist ensures quality case management services that assist youth in achieving their goals while exiting foster care.

Essential Functions

Conducts needs assessments for PAL clients and connects them with appropriate resources.

Advocates with human services, health, and court/criminal‑justice systems.

Provides crisis de‑escalation and develops plans to move clients out of state crises.

Creates detailed Individual Service Plans and meets regularly to assess progress and update plans.

Maintains timely and accurate file documentation and data entry for the PAL program.

Follows DFPS guidelines and policies regarding PAL TFSS services.

Demonstrates knowledge of all contracts and agency policies.

Measurable Deliverables

Meet all federal and state regulatory guidelines and standards.

Lead PAL program weekly and monthly events and activities.

Work evenings, weekends, and holidays as needed.

Implement BCFS safety protocols, including evacuations with children.

Maintain confidentiality in all service and program areas.

Uphold BCFS HHS professional and ethical standards, including respectful conduct and dress‑code compliance.

Respond to front‑lobby setup, field calls, emails, and visitors daily.

Attend all required trainings.

Maintain operational office equipment and supplies.

Respond sensitively to cultural and socio‑economic characteristics.

Communicate effectively in writing and verbally.

Work efficiently in a fast‑paced environment.

Maintain required computer literacy.

Work independently and as part of a multidisciplinary team.

Travel as needed.

Perform other duties per grant requirements.

Requirements

Maintain required certificates, including First Aid and CPR.

Valid Texas driver’s license with clear driving record and reliable transportation.

Pass pre‑employment drug screen and random drug tests.

Provide proof of work eligibility upon request.

Undergo pre‑employment and biennial criminal background and motor‑vehicle checks (fingerprints if applicable).
