Community Support Specialist II Adults

This is a
40-hour per week
Community Outreach Case Worker position.
You will assist clients by linking them to community resources such as housing and food assistance, and will work outdoors in the communities with clients daily (not inside the agency).
Local travel is required and you must have your own reliable personal vehicle with valid insurance.
You must hold a valid CLASS E or CLASS D driver’s license; if you do not, you must obtain one within two weeks prior to your start date.
You will be asked to transport clients in your personal vehicle and any mileage incurred while working in the community will be reimbursed.
Responsibilities
Assist clients by linking them to community resources (housing, food assistance, etc.).
Meet clients face‑to‑face and in their homes, providing community support services to complex clients.
Maintain a caseload that may vary; complete timely documentation of services, describing the need, intervention, relationship to the treatment plan, provider, date, time, location, and client response.
Contact individuals and referral sources following missed appointments to re‑engage and promote recovery/resiliency efforts.
Support individuals in crisis situations.
Provide mentorship and job shadowing to community support colleagues.
Participate in training during the first 90 days to prepare for work with individuals suffering from severe mental health issues.
Preferred Qualifications
Experience with individuals suffering from mental health challenges strongly preferred (NOT REQUIRED).
Ability to learn new things quickly and strong ability to use technology.
Minimum Requirements
Education: Bachelor’s Degree – Human Services or related.
No prior experience required.
License: Class D (IL) or Class E (MO).
