MDS Nurse Coordinator
Join CareCore Health LLC at Margaret Hall, a CMS‑rated 5‑Star facility. We are seeking a dedicated and detail‑oriented MDS Coordinator to join our leadership team. This is a full‑time, onsite role (not remote) where your clinical expertise and attention to detail will directly impact resident outcomes and facility success.
Responsibilities
Lead and manage the Resident Assessment Instrument (RAI) process with accuracy and timeliness
Ensure compliance with federal and state regulations through precise completion of MDS assessments
Partner with the interdisciplinary team to enhance care planning and quality measures
Monitor and maintain documentation to support optimal resident care and facility performance
Qualifications
Current RN license in good standing (required)
RAC‑CT certification preferred
In‑depth knowledge of MDS/RAI process and regulations
Strong organizational, leadership, and communication skills
Ability to collaborate across multiple disciplines
