The Community Engagement Team is designed to conduct outreach and in‑reach to identify and engage high‑acuity clients to be seen for intake and opened as new client at CCBHC programs.

The Case Manager provides individual and group skills training, case management, and coordination of multiple service providers for prospective clients under the direction of a senior clinician. Services are primarily off‑site (within the community and at partner locations). Keeps accurate and timely records of activities.

Location:

LifeWorks NW's Rockwood site.

Pay/Benefits:

$24.92–$32.09 per hour plus full benefits.

Salary Placement Notice:

The posted salary range reflects the compensation potential for this role. Most candidates will be placed between the entry level and midpoint of the range, depending on their experience and qualifications. More seasoned professionals may receive placement toward the higher end. Final salary offers are made in alignment with internal compensation processes in accordance with Oregon Pay Equity laws.

Essential Responsibilities

Responsible for client orientation including advising clients and significant others of the services offered and recommending services.

Provides case‑coordination by maintaining contact with other agencies and referral sources regarding client involvement in treatment services.

Conducts individual and group skill training sessions based on evidence‑based practices such as collaborative problem solving, cognitive behavioral interventions, and solution focused therapy.

Teaches socially acceptable behavior, employing techniques such as behavior modification and positive reinforcement; encourages individuals and families to develop and use skills and strategies for confronting problems constructively.

Observes and influences clients’ behavior, communication and interaction; detects unusual behavior and reports observations to professional staff.

Organizes, supervises, and encourages clients to participate in social, educational, and recreational activities; provides assistance with job search.

Assists the client to the extent necessary, based on functioning level, to access needed services and resources; advocates on client’s behalf; may attend psychiatric medical appointments if indicated.

Reviews and evaluates clients’ progress relative to measurable goals in treatment plans.

Provides clients or family members with information about mental health and addiction issues and about available services and programs; makes referrals when necessary.

Reports pertinent information to authorities in cases of endangerment, neglect, or abuse of children, severely ill individuals, or the elderly.

Acts as client advocate to coordinate required services or resolve emergency problems in crisis; provides clinical support to clients with immediate needs.

Assists with team tasks, such as arranging transportation.

Maintains chart notes related to external contracts, obtains records from other providers, and tracks client involvement and progress; completes documentation in electronic health record system timely.

Transports clients in personal or agency vehicles and travels between sites or in the community.

Qualifications

Valid and active QMHA credential certification through MHACBO.

Bachelor’s degree in psychology, counseling, social work, or a mental health related field, OR a minimum of 3 years of prior experience with the related population.

Demonstrated ability to communicate effectively; understand mental health assessment, treatment, and service terminology and counseling techniques.

Demonstrated ability to provide culturally competent treatment services to diverse populations and maintain cooperative relationships in a culturally diverse environment.

Demonstrated ability to provide psychosocial skills training and implement treatment plan elements.

Demonstrated ability to provide case management services for people with severe and persistent mental illness in compliance with confidentiality policies.

Knowledge of community resources and referral pathways.

Proficiency in Microsoft Outlook and Word; ability to master proprietary software including electronic health record and scheduling systems.

Requires access to a car, a valid driver’s license, proof of current automobile insurance coverage, and verification of safe driving record from DMV.
