Day Habilitation Coordinator
Under the supervision of the HCBS Director, the Day Habilitation Coordinator oversees the day-to-day operations of the Day Habilitation Program and serves as the introductory link to potential program participants.
Essential Functions
Reports directly to the Director of HCBS.
Assures all emergency procedures are implemented as required by Federal, State, Agency and Program regulations, policies, and procedures.
Schedules and conducts monthly Day Habilitation staff meetings.
Ensures Day Hab staff is up to date on all trainings.
Conducts monthly meetings with Nurse to ensure medical needs are met.
Ensures the development of the individual’s service planning process.
Ensures Programs meet State and Federal Regulations.
Ensures billing documents are completed according to Medicaid Standards.
Communicates and coordinates with nursing staff to ensure consumer's medical needs are met.
Establishes and maintains good working relationships with funding sources and other service delivery agencies.
Ensures full enrollment in the Day Habilitation by recruitment of new program participants and processing of required paperwork.
Meets with Community Resources to secure volunteer opportunities for Day Habilitation participants.
Connects individuals with community resources for volunteers, according to the participant skill level, strengths, and desire.
Conducts intake and pre-admission assessment in conjunction with the intake committee.
Organizes and implements activities that facilitate involvement of employers, potential program participants, and other DD providers.
Ensures the provision of all service plans for participants.
Conducts fire drills as per regulation and stays abreast with the latest fire safety practices.
Submits monthly and quarterly reports pertaining to program participant and program progress.
Responsible for submitting work orders as needed.
Attends trainings and meetings assigned by supervisor.
Recruits, hires, and supervises all Day Habilitation Staff.
Performs any other duties as assigned by supervisor.
Authority This position holds a relatively high degree of independence within the framework of the agency's policies and procedures. The individual must be resourceful, display initiative, understand management systems and interpersonal relations, and keep the immediate supervisor apprised of issues requiring higher-level intervention.

Skills and Abilities
Associates in Human Services or related field.
Experience working with OPWDD regulations and policies.
Experience working with individuals with developmental disabilities.
Medication certification per OPWDD regulations must be kept up to date.
Must possess a valid NYS driver's license and meet Ibero's Vehicle Operator Requirements.
A combination of training and experience other than the specified, if judged to be adequate for the job, may be accepted by the President and CEO.
Physical Demands The position does require occasional standing, squatting, lifting of up to approximately 30 lbs, and frequent sitting.
