Peer Support Specialist Optum is a global organization dedicated to improving health outcomes by connecting people with the care, pharmacy benefits, data and resources they need. The Peer Support Specialist works with the clinical team to implement care coordination, help stabilize consumers and reduce higher‑level services such as ER and inpatient psychiatric admissions.

Location and Schedule Queens, NY and surrounding boroughs. 75% local travel, mileage reimbursed. Monday‑Friday, 8:00 a.m.‑5:00 p.m. Eastern Standard Time. Field‑based position with optional home‑based work.

Primary Responsibilities

Use personal lived experience and training to help families and consumers define recovery goals.

Collaborate with Care Managers, providers, and community agencies to facilitate access to and transition between services.

Participate in care coordination, case planning, and crisis planning (WRAP).

Offer peer support during critical points in treatment and during discharge.

Document all interactions in the documentation platform.

Advocate for consumers and families, assist with grievance processes, and provide support for appointments.

Work independently, often without established procedures.

Perform other duties as assigned.

Required Qualifications

High School Diploma/GED (or higher).

Active, current Certified Peer Recovery Specialist or Certified Peer Support Specialist certification in New York or ability to obtain within 12 months.

1+ year of experience with community resources and Medicaid populations.

1+ year of experience applying principles of recovery and resiliency, with personal lived experience.

Proficiency with Microsoft Office (Word, Excel, Outlook) and multiple web applications.

Access to a quiet workspace at home that can secure PHI.

High‑speed internet connection and reliable transportation with a valid New York driver’s license.

Residence within commuting distance to Queens, NY.

Ability to travel up to 75% of the time within 60 miles of assigned territory.

Preferred Qualifications

Associate’s degree or higher.

Experience facilitating training events.
