Through multidisciplinary services clients can choose from medication management, therapy, group therapy, health and wellness support, employment services, and transportation support to develop skills and manage health.

What You’ll Do
Maintain and interact with a caseload of clients with diverse characteristics either in‑person or over the phone.
Facilitate each client’s achievement of long‑range outcomes of symptom management, employment and independent living based upon the needs of the client.
Engage difficult to serve clients by investigating all potential resources to gain access and make connections.
Help remove barriers for clients, which may include helping clients participate in community or social service programs and referring clients to educational or working programs.
Assist clients with transportation barriers, including transporting them in your personal vehicle to appointments and area resources.
Collect and maintain information about clients and document interactions within ReDiscover’s electronic medical record.
Communicate and collaborate with an internal multidisciplinary team to address aspects of wellness and ensure positive communication regarding client care coordination.
Requirements Of The Position
4‑year degree or equivalent experience.
Valid automobile driver’s license, appropriate insurance, and reliable registered vehicle available during work hours, including transporting clients in a personal vehicle.
At least two years of clinical experience providing direct service to children and adolescents in a mental‑health setting (preferred).
Knowledge of community resources in the Kansas City metro area and skill in establishing relationships with community partners (preferred).
Excellent customer service skills with a recovery focus.
Strong written, oral and interpersonal communication skills.
Intermediate computer skills in typing/keyboarding, Microsoft Word and Excel, and email usage.
