Care Management Assistant - Acute Case Management
Location & Schedule:
Akron Campus Acute Case Management 8:00am-4:30pm, Monday-Friday, Every 7th Weekend Shift
Summary:
The Care Management Assistant (CMA) provides support to members of the Care Management team, participates in discharge planning, care coordination, and transition planning. The CMA supports efficient movement of patients to post‑acute facilities including Skilled Nursing Facilities, Long‑Term Acute Care Hospitals, Inpatient Rehab Facilities, and Intermediate Care Facilities by creating referrals, transmitting via HIPAA secure platform, and initiating or triggering pre‑certification as directed.
Additional responsibilities include compliance with federal and state regulations, serving patient rights notifications related to Discharge Planning and Utilization Management, working with interdisciplinary teams, assisting with transitions of care, arranging outpatient follow‑up, and collaborating with community providers, facility admissions staff, and insurance companies. The role requires initiative, proficiency, strong decision‑making, organizational and communication skills, and a commitment to quality service.
Minimum Qualifications
Associate degree or equivalent combination of education and experience
Preferred Qualifications
Basic experience utilizing Microsoft Office products
Knowledge of medical terminology
One year of experience working with electronic health record (EHR)
Previous insurance and prior authorization experience
Skills & Competencies
Ability to prioritize and maintain excellent organizational skills
Ability to work in a fast‑paced environment with attention to detail
Ability to complete multiple tasks simultaneously
Maintains confidentiality of all patient information
Demonstrates basic computer skills and navigation of electronic devices and platforms
Knowledge of CPT and ICD‑10 codes
Interpersonal skills to interact effectively with patients, families, interdisciplinary caregivers, peers, subordinates, and supervisors
Excellent verbal and written communication skills
Ability to effectively interact with patients/customers with respect and dignity
Physical Demands
Mostly sedentary; may occasionally lift up to 10 lbs
Salary Hourly rate: $19.23 – $23.08 per hour (base salary only, no bonuses or differentials). Salary determined by experience and location. Pay equity considerations applied.
