Youth Transition Specialist (Independent Living)
TAG MedStaffing is hiring a Youth Transition Specialist (Independent Living) in Bronx, NY for our client, supporting transition planning and independent living skill development for youth and young adults. In this temporary role, you will coordinate transition activities, facilitate life‑skills workshops, maintain documentation, and collaborate with internal teams and community partners.
Compensation:
$28/hour
Location:
Bronx, NY
Schedule:
Monday to Friday (35 Hours/Week)
Responsibilities
Transition Planning & Program Oversight: Oversee transition planning activities, coordinate discharge meetings, and support youth preparation for adulthood and self‑sufficiency.
Independent Living & Skill Building: Plan and facilitate workshops focused on life skills including housing, budgeting, health and wellness, education, employment readiness, and community resources.
Documentation, Compliance & Reporting: Maintain accurate records, complete required checklists, support compliance efforts, and ensure timely documentation and reporting.
Youth Engagement & Collaboration: Engage youth through outreach and collaboration with case planners, specialists, and community partners to support education, employment, and independent living goals.
Qualifications
Education: Bachelor’s degree in Social Work, Psychology, Education, Human Services, or a related field required.
Experience: 2+ years of experience working with adolescents or young adults, preferably within child welfare, foster care, or youth development programs.
Technical Skills: Proficiency in Microsoft Word and Excel with the ability to learn case management, tracking, and reporting systems.
Industry Knowledge: Familiarity with independent living skills development, transition planning, and youth‑serving systems and programs.
Skills & Attributes: Strong facilitation and communication skills with excellent organization, sound judgment, and a strengths‑based, youth and family development approach.
Language Skills: Bilingual in English and Spanish preferred.
