Case Management Coordinator

Simple Solutions Psychotherapy - Founded June of 2020. We are a community-based organization providing therapy (individual, family, couples, & groups), after-school programs, mental health assemblies, organizational workshops, juvenile detention centers, case management and more! We service California residents via insurance or cash pay. Services are provided via telehealth and in-person in English and Spanish Language and based on specifics of contracts.

At Simple Solutions Psychotherapy we believe "It's SIMPLE people just make it complicated." We seek to assist PsyD practicum students in gaining knowledge and experience toward assisting the community with mental health services.

Case Management Coordinator

to join our team! If you are passionate about helping clients navigate their health, social, and psychological needs while coordinating care and resources, we want to hear from you.

What You’ll Do:

Conduct thorough client assessments across health, social, and financial needs

Develop and implement individualized care plans

Coordinate services and support for clients and their families

Document all client interactions accurately and efficiently

What We’re Looking For:

Must have a Masters Degree in Social Work.

Experience in case management, social work, or a related field

Compassionate, proactive, and client-focused mindset

The pay range for this role is: 20 - 25 USD per hour (Rancho Cucamonga, CA)
