We provide a range of comprehensive services aimed at supporting young people and their families to overcome challenges and thrive. We are seeking a dynamic and passionate Housing Case Manager to join our team.
What you’ll do as a Housing Case Manager
Provide individualized case management, including safety and needs assessments, counseling, advocacy, referrals, and coordination with community services Maintain accurate case documentation and reports, and manage data in HMIS and Evergreen databases Build and maintain strong professional relationships with clients, Evergreen team members, and community partners Assist youth with housing searches, applications, and navigating landlord-tenant responsibilities Support youth through housing transitions and prevent evictions when possible Conduct skills-building groups and participate in outreach, advocacy, and community education Share in reporting for funders, board updates, and agency needs Other duties as assigned to support Evergreen’s mission What we’re looking for
