Description We are currently seeking Wound Care Nurse Practitioners to provide wound services to patients living in long‑term care facilities.
Role and Responsibilities
Provide wound care services for long‑term care patients residing in skilled nursing facilities.
Prescribe appropriate medications, including controlled substances in accordance with PA Law.
Order, review, and/or follow‑up with treatments and diagnostic testing.
Communicate with the nursing home staff on a regular basis and as needed to discuss changes in condition and the care plan.
Collaborate with the primary care physician.
Qualifications and Education Requirements
Current, unrestricted NP license in the state of Pennsylvania.
Board Certification.
DEA licensure (Optional)
Clinical background in adult, family, or geriatric specialties.
Clinical background of wound care, including but not limited to chronic, debilitating, stalled, or infected wounds.
Comfortable diagnosing and treating diabetic, arterial, venous, and mixed etiology wounds or Kennedy Ulcers.
Comfortable staging pressure injuries according to the NPIAP.
Familiar with wound care best practice and advanced wound care products; collagens, alginates, foams, silvers, etc.
Experience with wound vacs and other DME, such as pressure‑relieving/reducing devices.
Strong preference for having wound certification or willingness to obtain certification.
Experience working in a long‑term care setting.
Ability to work with an EMR.
Ability to work within an interdisciplinary care team to coordinate patient care.
