What You'll Do:
Provide skilled physical therapy services in patients' homes
Evaluate mobility, strength, and functional limitations
Develop and implement individualized treatment plans
Educate clients and caregivers on home exercise programs and mobility techniquesMonitor progress, document outcomes, and adjust treatment as needed
Collaborate with the interdisciplinary care team to ensure quality, coordinated care
What You'll Need:
Active
Physical Therapist (PT) license
in Florida
Degree from an
accredited Physical Therapy program
Valid driver's license and reliable transportation
Current
CPR certification
Strong communication, documentation, and time-management skills
Home health experience preferred
