Summary / Objective
Windsor Rehab is seeking a dedicated and experienced
Wound Care Nurse
to join our clinical team. This role is essential in improving resident outcomes through high‑quality wound care, accurate assessments, and proactive prevention. The Wound Care Nurse will collaborate with physicians, supervise staff, and ensure compliance with all clinical standards, policies, and regulations. This position is ideal for a Wound Care Nurse who is passionate about resident care, wound prevention, and clinical excellence. Essential Functions
Daily Responsibilities
Complete head‑to‑toe skin audits on all new admissions and re‑admissions. Ensure skin sheets, treatment orders, and preventative measures (T&P, specialty mattresses, heel protection) are completed. Review new skin alterations and ensure all required documentation and interventions are implemented. Weekly Responsibilities
Update skin sheets every seven days. Ensure weekly care plan updates for pressure and non‑pressure wounds. Submit weekly wound report to VP of Clinical Services. Perform wound treatments for vascular, surgical, and stage 2+ pressure wounds Monday through Friday. Oversee charge nurse completion of skin tear documentation. Ongoing Responsibilities
Provide daily wound‑prevention education to staff. Adjust treatment plans within two weeks if wounds worsen. Ensure preventative measures such as T&P orders, barrier creams, cushions, and specialty mattresses. Participate in interdisciplinary rounds to ensure proper positioning, heel elevation, and skin protection. Additional Duties
Collaborate with nursing leadership on assessments and treatment planning. Initiate physician consults when needed. Assess residents on admission and identify abnormal findings. Supervise CNAs and nursing staff to ensure quality care. Administer medications within RN/LPN scope. Complete accurate charting and monthly evaluations. Ensure care plans are followed and updated. Round with wound physician weekly. Maintain infection control and safety standards. Promote resident dignity, comfort, and privacy. Assist with admissions as needed. Participate in meetings and in‑services. Required Education and Experience
Graduate of an accredited nursing program Current RN or LPN license in the State of North Carolina CPR certification Wound Care Certification preferred Why Work at Windsor Rehab?
