Setting
Outpatient Clinic working with the geriatric population.
Responsibilities
Conduct social work assessments to identify the psychosocial needs, preferences, and goals of patients, contributing to the development of personalized care plans.
Actively participate in team meetings to collaborate on care plan creation and patient management.
Provide social work interventions as outlined in patient care plans, including resource arrangement, care transitions, individual and group counseling, and case management.
Partner with primary care physicians and other team members to ensure smooth transitions of care between settings, such as hospitals, skilled nursing facilities, and home care.
Coordinate, facilitate, and lead care conference meetings to ensure effective communication and care coordination with patients, families, caregivers, and team members.
Offer discharge planning and support when patients disenroll from the program.
Other duties as assigned.
Qualifications
Master’s Degree in Social Work (MSW).
A minimum of one (1) year of experience working with a frail or elderly population.
Experience coordinating and facilitating care conference meetings.
Preferred experience working with individuals who have behavioral health and substance abuse issues.
