Responsibilities At Alexian PACE, we provide a Program of All‑Inclusive Care for the Elderly (PACE). We are seeking to add a BE/BC Primary Care Physician to join our PACE family at our Chattanooga, TN location.
Job Duties:
Working Full Time seeing clinic based patients
Providing routine assessments to elderly patients. Significant change assessments; Hospital follow ups; and/or Acute visits
Ability to supervise APPs
On occasion, visit might involve traveling to an Assisted Living or Nursing Home to see a patient
Call coverage is 1:6 (No hospital visits)
Seeing providers with great communication skills and a desire to work with the Elderly
Our average providers has more than 10 years of service within the PACE family
CME (Days/Dollars) available for our providers

Ascension Living Alexian PACE is a community‑based program whose purpose is to serve the frail elderly residents of Hamilton County with a healthcare model including senior home care, adult daycare and specialized geriatric medical care.
PACE is a coordinated health and social services program that enables participants to remain in their own homes and communities rather than moving to nursing homes. It is a unique system of managed care which includes services in the home, at the PACE center, and if necessary, in the hospital or other medical facility. PACE serves the frailest older adults with top quality healthcare and helps people age with dignity at home.

Requirements Licensure / Certification / Registration:
Required Credential(s):
Physician MD/DO obtained prior to hire date or job transfer date.
Preferred Credential(s):
Advanced Life Support.
Education:
Doctor of Medicine (MD) or Doctor of Osteopathy (DO) required.
