Responsibilities
Complete an initial, comprehensive, and ongoing assessment of patient and family to determine hospice needs. Provide a complete physical assessment and history of current and previous illnesses.
Complete initial evaluations, consents, and admissions for patients.
Coordinate patient care for assigned caseload.
Provide professional nursing care by utilizing all elements of the nursing process.
Assess and evaluate patient’s status.
Write and initiate plan of care.
Re‑evaluate patient and family/caregiver needs regularly.
Participate in revising the plan of care as necessary.
Initiate the plan of care and make necessary revisions as patient status and needs change.
Use health assessment data to determine nursing diagnosis.
Develop a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Include the patient and the family in the planning process.
Initiate appropriate preventive and rehabilitative nursing procedures. Administer medications and treatments as prescribed by the physician in the physician’s plan of care.
Counsel the patient and family in meeting nursing and related needs.
Provide health care instructions to the patient as appropriate per assessment and plan.
Assist the patient with activities of daily living and facilitate the patient’s efforts toward self‑sufficiency and optional comfort care.
Must complete weekly visit minimum as established by Office Administrator.
Qualifications
Current Registered Nurse (RN) licensure (in good standing) in the state(s) of practice.
Works well within a team environment to collaborate daily with nurses, social workers, chaplains, volunteers, and CNA leadership.
Work closely with patients on a 1‑1 level but get strong support from a patient care team.
Can work independently and on your own from a day‑to‑day basis with activities which include phone, email and written communication, meetings with a variety of healthcare professionals, and an understanding of hospice rules and regulations.
Time management skills to manage visits, documentation, meetings (i.e. to scheduled visits, attend required meetings, respond to urgent needs by prioritizing and triaging multiple concerns)
Ability to learn an electronic medical records system.
Must be willing to drive with reliable transportation, valid driver’s license, and auto insurance, and be able to travel across the service area.
