Role Overview A Non‑Medical Care Manager contributes to AHF by utilizing strong experience in social work, understanding the social deterrents of this population, and a thorough understanding of the Ryan White Care Act Program within their specific area. Working within the Healthcare Center, the Manager completes Health Risk/Needs Assessments, develops Service/Action/Care Plans related to psychosocial needs, and facilitates referrals to programs and agencies that address basic needs such as housing, food and clothing. The Manager also provides education and assistance to ensure patients understand program eligibility and supports them in achieving healthcare goals.
The role enhances AHF’s reputation by meeting members where they are, identifying strengths and opportunities for growth, and engaging them with realistic goal setting. Through collaboration and engagement, the Manager brings the essence of the Ryan White Care Act—the safety net for uninsured HIV/AIDS patients—to our most vulnerable population. The Manager’s work supports deliverables of the Ryan White contract and contributes to measures established by the HIV/AIDS Bureau and the Department of Health and Human Services.
Qualifications
Bachelor’s degree in health or human services‑related field (preferred).
Minimum of 1 year case‑management experience (preferred).
Knowledge of Ryan White Services (preferable).
