Overview Our team is expanding and we are hiring PRN Physical Therapists for our innovative Home and Community program. This position is for our weekend team, with PRN opportunities available on Saturdays, Sundays, or both.

Responsibilities
Perform evaluations and develop treatment plans
Regularly reassess effectiveness of treatment plans, attend staffing meetings and family conferences
Perform discharge evaluations, make appropriate recommendations for home and community safety, and provide referral and educational resources
Communicate patient’s needs and progress to the treatment team, physician, person receiving services, and family members
Communicate with other disciplines to ensure collaboration, coordination of care, and enhance patient outcomes
Qualifications
Minimum bachelor’s degree in Physical Therapy from an accredited program
Current, unrestricted license as Physical Therapist in the state of practice
Current Basic Life Support or Cardiopulmonary Resuscitation certification per state regulations
Preferred: a minimum of one year’s work experience as a physical therapist
Knowledge of rehabilitation techniques related to complex neurological injury
Effective verbal and written communication skills
Ability to lift 50 pounds and handle patient movements involving up to 100 pounds following safety procedures
Physical requirements: fine motor skills, visual acuity, extended walking/standing, and frequent pushing/pulling, bending/stooping, reaching, kneeling, and positioning
Health screen or examination may be required prior to assignment and periodically thereafter to verify physical capability

Salary Range USD $55.00 – $60.00 per hour
