Job Description Summary In partnership with the Office of Diversion and Re‑Entry (ODR), the Support Specialist at Mollie Maison will function as part of a multidisciplinary team and provide site‑based therapeutic and intervention services to clients residing in Project‑Based Housing. These clients have been involved in the criminal justice system and, through ODR, are receiving ongoing case‑management services to obtain and maintain permanent supportive housing in the community.
Services provided by the Support Specialist include supporting the LVN by performing various care activities and aiding with activities of daily living (ADLs). The candidate will also assist Case Managers in working toward goal plans to eliminate barriers to housing and health stability, monitor daily site activities, and conduct wellness groups to foster community and life‑skills development.
Essential Duties and Responsibilities
Support orientation to permanent housing for new clients.
Oversee housekeeping and hygiene guidelines in a trauma‑informed manner.
Work with LVN to support medical intervention plans.
Collaborate with Case Managers to ensure proactive client care.
Assist with meal support or other duties as assigned to maintain site operations.
Order office and program supplies when needed.
Facilitate client groups to assist with life‑skills development.
Develop trusting relationships with clients using motivational interviewing, harm reduction, and trauma‑informed care.
Create an inclusive environment to support clients' permanent housing retention and psycho‑social rehabilitation.
Work with TPC leadership and supervise staff to develop relationships with property management.
Assist Property Management with client‑related requests when appropriate.
Intervene during crises to de‑escalate situations, supporting safety for staff and clients.
Communicate effectively and timely with management, coworkers, and clients.
Maintain client confidentiality per agency, state, and HIPAA laws.
Report all statistics and outcome updates for reporting purposes.
Attend case conferences, staff meetings, and training sessions.
Perform other duties as assigned.
Qualifications
High School Diploma, GED, or equivalent.
Minimum of one year providing services to individuals who have a history of homelessness, mental illness, or dual diagnoses.
Experience working in an interim housing site is a plus.
Proficient in Word, Excel, and Outlook.
Obtain and maintain CPR and first aid certification.
Demonstrated knowledge of issues faced by a culturally diverse population.
Work Environment
Field and indoor office environment; may need to travel.
Occasional travel to local sites.
Regularly required to sit, stand, bend, and lift up to 35 lbs.
Work in busy, at times loud environments.
Exposure to elements such as cold, heat, dust, noise, and odor.
Work requires frequent bending, stooping, twisting.
Team‑oriented, collaborative environment.
