Overview
Community Health Worker role at AbsoluteCare Connect members with resources to address social needs and overcome barriers to care. Build trust and relationships at home, community, hospital, office. Educate new members and guide enrollment in internal and external services. Duties and Responsibilities
Represent AbsoluteCare and communicate how services, connections, and resources help in member’s specific situation. Conduct in-person non-clinical assessments to understand member needs, preferences, and social barriers to care and evaluate home environment as needed. Connect members with appropriate AbsoluteCare and community resources such as mental health, substance abuse, and housing support. Build trust and rapport with members, support systems, and communities to drive engagement. Partner with care team (community, providers, internal staff) and attend daily huddles and team meetings to prioritize members, caseload, and align actions. Explore new and innovative ways to engage current members and locate lost members. Document daily outreach activities into CRM and electronic health and/or medical record. Meet predefined metrics and KPIs for member engagement. Accompany members to primary care physician visits to improve health outcomes and reduce hospital stays. Support members within the Member Lounge with events. Minimum Qualifications
High school diploma or equivalent required. Valid driver’s license, proof of insurance, and reliable personal vehicle. 2+ years experience working with community agency providing health care services. Certified CHW preferred. Experience engaging vulnerable, hard-to-engage, complex populations. Independent and persistent self-starter. Proficient PC skills, such as Microsoft Office. Ability to travel locally 80% of the time. Knowledge of and relationships within local community and experience navigating community resources. Working Conditions
Operate in community, member homes, and clinical center. Dedicated quiet workspace adhering to HIPAA. Reliable high-speed Wi‑Fi. Personal vehicle required for outreach. Physical Requirements
Drive and walk in community. Communicate clearly and exchange accurate information. Remain stationary for long periods. Operate computer, keyboard, copy, fax, phone, and other general office equipment. Occasionally move objects up to 20 lbs.
