Transitions At Home, is currently seeking to hire a Full-Time Physical Therapist. We are seeking a compassionate and driven Physical Therapist to provide personalized, one‑on‑one care in patients’ homes—helping them regain strength, mobility, and independence. Physical Therapist Responsibilities An Physical Therapist (PT) administers physical therapy to patients on an intermittent basis in their place of residence. This is performed in accordance with physician orders and plan of care under the direction and supervision of the Director of Clinical Services/Clinical Manager.
Hours 8:00am – 4:30pm Monday through Friday No Holidays

Qualifications / Job Requirements Active Wisconsin Physical Therapist License Two (2) years’ experience preferred. CPR Certification A valid Wisconsin Driver's License and Car Insurance
