Responsibilities
Provide skilled nursing care and assessments to patients in their homes.
Develop, implement, and evaluate individualized care plans.
Administer medications and treatments as prescribed by physicians.
Monitor patient progress and communicate any changes to the healthcare team.
Educate patients and their families on disease management and health maintenance.
Maintain accurate and timely documentation of patient care.
Coordinate with interdisciplinary team members to ensure continuity of care.
Requirements
Valid Registered Nurse (RN) license in the state of Michigan.
Excellent clinical assessment and critical thinking skills.
Strong interpersonal and communication abilities.
Ability to work independently and manage time effectively.
Reliable transportation to travel to patient homes within the designated area.
