Position Overview We are seeking an
experienced Home Health Physical Therapist Assistant (PTA)
to support patients in their homes. This role is ideal for a motivated PTA who values independence, collaboration, and making a real impact on patient mobility and quality of life.
Key Responsibilities
Assist the supervising Physical Therapist with patient evaluations and ongoing assessments
Carry out prescribed treatment plans, therapeutic exercises, and mobility training
Provide hands‑on assistance with gait training, transfers, balance, and ADLs
Educate patients and caregivers on home exercise programs and safety techniques
Monitor patient progress and report changes to the Physical Therapist
Document visits accurately and timely per regulatory and agency standards
Collaborate with the interdisciplinary care team to ensure continuity of care
Qualifications
Associate’s degree from an accredited
Physical Therapist Assistant program
Current
Virginia PTA license
or eligibility
Minimum
2 years of experience , preferably in home health, rehab, or geriatrics
Strong clinical, communication, and documentation skills
Knowledge of anatomy, kinesiology, and physical therapy principles
Valid driver’s license and reliable transportation
