Job Summary Grove Park Healthcare Center, is seeking a dedicated and compassionate LPN Licensed Practical Nurse to join our team at our state-of-the-art facility in East Orange, New Jersey. As a member of our interdisciplinary care team, you will play a vital role in providing exceptional patient care and support to our residents.
Job Responsibilities
Provide direct patient care, including administering medications, wound care, and vital sign monitoring
Monitor and report patient conditions to the supervising physician and other members of the healthcare team
Develop and implement individualized care plans in conjunction with the interdisciplinary care team
Participate in care planning and discharge planning for residents
Maintain accurate and detailed records of patient care and treatment
Collaborate with other healthcare professionals to ensure quality and coordinated care
Demonstrate a commitment to delivering compassionate and empathetic care to our residents
Requirements
Current LPN licensure in the State of New Jersey
Strong communication and interpersonal skills
Ability to work effectively in a fast-paced environment
Commitment to delivering high-quality patient care
