Case Manager Generalist Volunteers of America Los Angeles seeks a Case Manager Generalist to provide comprehensive case management services to low‑acuity clients.

Pay Rate $22.91 - $24.12 Per Hour

Responsibilities

Conduct comprehensive screenings and assessments with individuals and families to collect functional, environmental, psycho‑social, financial, employment, housing, educational, and health information.

Assess barriers, develop a detailed case plan, coordinate individualized planning to meet short‑ and long‑term needs, and connect clients to service linkages and supportive systems.

Coordinate case conferences, consult with the service team, monitor progress, and assist clients to meet agreed‑upon goals.

Advocate for clients before judicial, community, social service, and administrative bodies; facilitate support groups and client participation as appropriate.

Essential Duties

Assessment: conduct regular individual meetings, develop a detailed plan of action, set goals, and secure resources.

Coaching and Social Support: accompany participants to activities and services.

Coordinate Care and Case Management: connect participants to appropriate services.

Advocacy: represent participants before other agencies and community settings.

Capacity Building: motivate participant progress.

Outreach and Engagement: meet participants and bring them into programs.

Qualifications

Must pass fingerprint clearance, background check, and verification of employment and education.

Education

HS/GED. Completion of VOALA Case Management training within six months of hire.

Experience

Minimum one‑year general experience in case management or similar social services role.

Or Bachelor’s Degree in Social Services related field in lieu of experience.

Preferred Qualifications

Bachelor’s Degree in Social Work, Social Services, public administration, public health or related field.

One year of experience working with target population.

HMIS navigation.
