Role overview:
Physical Therapist evaluates and treats residents under orders from a physician, using therapeutic procedures and assistive devices to maximize functional potentials. Participation as a member of the interdisciplinary team providing support and information within the area of Physical Therapy.
Responsibilities:
Perform assessments, develop treatment plans, and deliver interventions for post-acute, home health, and SNF patients.
Collaborate with physicians and interdisciplinary team to optimize patient outcomes.
Maintain accurate documentation and progress reports.
Adhere to company and regulatory standards for therapeutic care.
Qualifications:
Licensed Physical Therapist in South Carolina.
Experience with residential and post-acute care settings preferred.
Strong communication and teamwork skills.
Commitment to patient-centered care and continuous learning.
