RN Recertification Nurse - Home Health Agency (Part Time or Full Time)
Home Care Network, Inc. (HCN) is a nurse‑owned and locally led home health agency that places patient care and wellbeing first. We are creating a unique position for an experienced home health Registered Nurse (RN) to provide recertifications and related services to clients across southeastern Ohio, primarily in Jackson, Vinton, Pike, and Gallia counties. The role offers a consistent schedule—no overnight shifts or on‑call duties—while allowing flexibility for part‑time or full‑time commitment. Job Summary
As an RN Recertification Nurse you will coordinate, re‑evaluate, and confirm the medical necessity of continued home health care services. Your responsibilities include performing patient assessments, completing documentation, ensuring compliance with state, federal, and agency guidelines, and communicating with physicians, agency staff, and clients. Responsibilities
Travel to client homes within a 1‑to‑2 hour radius on scheduled days (per week). Provide recertifications and, as needed, start‑of‑care (SOC), resume‑of‑care (ROC), and other assessments. Maintain accurate OASIS and non‑OASIS documentation and submit updates promptly. Collaborate with physicians, service administrators, and other caregivers to coordinate care plans. Adhere to compliance and quality standards, and assist in new‑hire orientation and supervisory reviews. Qualifications
Graduate of an accredited School of Nursing. Minimum one year of RN experience and an active RN license in Ohio. Minimum two years in a home health or equivalent setting. CPR and First Aid certifications (or willingness to obtain before hire). Experience with OASIS and non‑OASIS assessments. Strong background in patient care, documentation, and confidentiality. Excellent attendance and dependability record. Familiarity with HIPAA regulations and medical terminology. Valid driver’s license and reliable transportation. Willingness to travel to multiple counties. Compensation
Hourly rate starting at $35/hr. Annual salary (with PTO) negotiable based on a minimum number of recertifications per week. Hourly pay paid weekly with potential overtime; salary negotiated for full‑time staff. Benefits and Perks
