Job Summary As a Physical Therapist (PT) at Woodstock Nursing & Rehabilitation Center, you will have the opportunity to work with a diverse patient population, from geriatric to adult, to help them regain strength, mobility, and independence. You will develop and implement individualized treatment plans, conduct assessments and evaluations, and collaborate with our interdisciplinary team to ensure the best possible outcomes for our patients. Responsibilities Conduct patient evaluations and assessments to identify treatment needs and goals Develop and implement individualized treatment plans to promote optimal functional outcomes Provide physical therapy interventions, including but not limited to: exercises, manual therapy, and modalities Contribute to the development and implementation of patient education programs Communicate effectively with patients, families, and healthcare team members Maintain accurate and detailed records of patient treatment and progress Requirements Bachelor's or Master's degree in Physical Therapy from an accredited program Minimum of 0 years of experience as a Physical Therapist (licensure or certification pending) Possession of a valid Physical Therapy license in the state of Georgia Ability to work collaboratively as part of an interdisciplinary team Excellent communication and interpersonal skills How To Apply If you are a dedicated and compassionate Physical Therapist looking for a rewarding career opportunity, please apply to this role by [insert call to apply].
