Job Summary
Under the supervision of a Registered Nurse, the Licensed Practical Nurse (LPN) provides direct and indirect nursing care to residents in a long‑term care setting. This role includes assessing resident needs, administering medications, performing treatments, and supporting residents with daily living activities. The LPN supports interdisciplinary care planning and ensures regulatory compliance while maintaining a focus on resident dignity and quality of life.
Essential Functions
Provides safe, competent nursing care under RN supervision in accordance with resident care plans.
Administers medications and treatments, including wound care and specimen collection.
Monitors and reports changes in residents’ health status to the physician and care team.
Documents all assessments, interventions, and responses accurately in the electronic medical record.
Assists residents with activities of daily living (ADLs) such as hygiene, elimination, nutrition, and ambulation.
Collaborates with interdisciplinary team to implement and evaluate care plans.
Maintains medication safety by following the six rights of medication administration.
Participates in resident and family education on health conditions and pain management.
Provides leadership to Certified Nursing Assistants (CNAs), ensuring continuity of care.
Responds to emergencies and communicates effectively with clinical and support staff.
Maintains regulatory compliance, infection control practices, and patient safety standards.
Supports orientation and training of new staff as needed.

Proficiency in EMR systems and basic computer applications.

Qualifications Education
Licensed Practical Nursing degree required.
Licenses & Certifications
Current LPN license or Compact State license.
Basic Life Support (BLS) certification required within 90 days of hire or transfer.
Experience
Experience in long‑term care or geriatric nursing preferred but not required.
