Job Summary

An Infusion Nurse II coordinates and delivers intermediate direct patient care to patients in the home or an alternate infusion suite. The role ensures patient safety, continuity of care, and compliance with a physician’s plan of care while working under structured supervision.

Job Responsibilities

Initiates, develops, and implements intermediate nursing plan of care treatments, evaluating patient progress toward goals.

Organizes and participates in the provision of direct patient care, performs treatments, administers medications, and educates patients and families/caregivers.

Modifies plan of treatment in response to changing patient status or physician orders to achieve established or revised patient care goals.

Assesses patient needs and physical status at each skilled visit through health data access and patient interview.

Re-evaluates patient needs through physical reassessment and supplemental physician orders.

Obtains and clarifies physician orders for plan of treatment revisions, informs physicians promptly of significant changes, and provides written summaries.

Provides training and mentorship to Infusion Nurse I’s as needed to ensure patient safety and compliance.

Teaches patients and families/facilitators patient‑centered care to promote independence with prescribed therapy and care needs.

Communicates effectively and timely with Option Care’s Clinical team, medical providers, patients, and families/caregivers to facilitate continuity of care.

Prepares and submits clinical documentation in real‑time during visits to meet billing optimization timelines.

Coordinates discharge planning, prepares discharge summaries, and reports patient progress and needs when transferring care.

Demonstrates compliance with agency operations, policies, procedures, regulations, and accreditation standards.

Maximizes work efficiency through technology to validate orders and track patient care.

Responds promptly to patient requests and initiates emergency procedures as necessary.

Accepts accountability for own practice through ethical and professional conduct.

Observes legal and ethical guidelines, safeguarding patient confidentiality and HIPAA compliance.

Provides financial instruction regarding Option Care’s services and charges.

Attends and completes required training, in‑services, and continuing education to maintain competency and licensure.

Provides oversight and coordination of paraprofessionals and may act as Supervisor in the absence of the Supervisor or Nurse Manager.

Participates in multidisciplinary team conferences and offers precepting, training, and mentoring to other nurses.

Participates in nursing department on‑call responsibilities.

Performs other related duties as directed by supervisor.

If Applicable

Inserts PICC/Midlines per physician order.

Troubleshoots/declots obstructed central line catheters.

Monitors PICC/Midline and educates on line infection prevention and routine care.

Supervisory Responsibilities No supervisory responsibilities such as hiring, promotions, scheduling, or performance reviews.

Basic Education And/or Experience

Active and unrestricted RN license in the state of practice.

Minimum of 1 year of nursing experience.

Current CPR certification required.

Basic Qualifications

Demonstrated competency in patient care standards and infusion skill sets required for safe service delivery.

Possession of a valid driver’s license and vehicle for business travel with proof of liability insurance.

Basic computer knowledge and the ability to use nursing technology tools for documentation and communication.

Physical Demand Requirements

Ability to lift up to 50 pounds (maximum 75 pounds) with occasional or frequent use of 10–20 pound force.

Work may involve walking, standing, crouching, kneeling, turning, pivoting, balancing, stooping, reaching overhead, grasping, pushing, pulling, lifting, and carrying.

Exposure to blood, body fluids, hazardous substances, sharps, chemicals, and unpredictable home environments.

Safety requirements include closed‑toe shoes, facemask, goggles, gown, gloves, and functional communication devices.

Travel Requirements

Willing to travel up to 100% of the time for business purposes.

Preferred Qualifications & Interests (PQIs)

Bachelor of Science in Nursing preferred.

1–2 years of prior infusion nursing experience preferred.

Salary and Benefits Salary to be determined by applicant’s education, experience, skills, internal equity, and market data.

Pay Range: $36.89–$61.49 per hour.
