Case Manager

Job Description Provide mental health, substance abuse, and outreach services to individuals who may have mental health and/or substance use disorders. Work with community partners and stakeholders to offer comprehensive care. Meet clients in office, hospital, or their own environment (home, schools/work, etc.). Ensure continuity of care, link clients and families to needed resources to improve quality of life. Clinical training opportunities available.
Essential Duties & Responsibilities
Reports to work as scheduled and performs duties as assigned by supervisor to ensure smooth operation.
Provides individual and group mental health and substance use disorder case management services; refers, advocates, and links clients and families to additional services.
Provides supportive and educational counseling to develop daily living and coping skills.
Transport clients in company vehicles to necessary appointments.
Develops, implements, evaluates, and updates treatment plans with client and family input.
Conducts clinical behavioral health assessments.
Documents services in Electronic Medical Record in a timely manner.
Acts as liaison and communicates with local law enforcement and stakeholders to coordinate treatment process.
Completes required agency trainings to maintain 100% compliance.
Maintains departmental productivity expectations.
Completes additional duties assigned.
Knowledge, Skills & Abilities
Effective communication via written, verbal, in person, and virtual methods.
Understanding of individuals with co‑occurring issues and their families.
Good organizational and time‑management skills.
Competency with electronic medical records, computer systems, and documentation.
Strong interpersonal communication skills and professionalism.
Knowledge of community resources or ability to learn quickly.
Ability to develop supportive relationships while maintaining therapeutic boundaries; must have a valid FL driver’s license.
Qualifications Education Level : Bachelor's degree required.
Certification/Licensure : Must have a valid FL driver’s license with a good driving record and current auto insurance.
