Job Description Summary An Infusion Nurse I is a licensed registered nurse who provides basic direct patient care to patients in the home or an alternate infusion site, ensuring patient safety with continuity and compliance under a physician’s plan of care while working under close direct supervision.
Job Responsibilities
Initiate, develop, and implement nursing plan of care treatments, evaluating patient progress toward goals; participate in direct patient care, administer medications and educate patients and families/caregivers.
Assess patient needs and physical status at each visit; re‑evaluate through reassessment and physician orders; obtain and clarify orders; notify physician of significant changes and provide written summary.
Provide effective and safe teaching to patients and families to achieve independence with therapy needs through active participation.
Communicate timely with Naven Health’s Clinical team, medical providers, patients and families to facilitate continuity of care; prepare clinical documentation in real time and comply with billing timelines.
Under close supervision, coordinate discharge planning and prepare discharge summaries with patient instructions and comprehensive reports when transferring care.
Demonstrate compliance with agency operations, Naven Health’s policies, professional standards, local, state, federal regulations and accreditation standards.
Maximize work efficiency using computers and automation technologies to validate orders, communicate patient care, and follow visit schedule.
Respond promptly to patient requests and initiate emergency procedures as necessary.
Accept accountability for practice through ethical and professional conduct following established programs and policies.
Observe legal and ethical guidelines to safeguard confidentiality of patient and proprietary information, including HIPAA compliance.
Speak knowledgeably about Naven Health’s scope of services and instruct patients about related plan of care.
Attend and complete required training modules, in‑services, and continuing education to maintain competency and licensure; provide oversight and coordination of paraprofessionals and act as supervisor in absence of the supervisor or nurse manager when applicable.
Participate in multidisciplinary team conferences and provide precepting, training and mentoring to other nurses for orientation and onboarding.
Participate in nursing on‑call responsibilities as applicable.
Perform other related duties as directed by supervisor.
Basic Education And/or Experience Requirements
Active and unrestricted RN license in the state of practice.
Minimum of 0–2 years of infusion nursing experience.
Current CPR certification.
Basic Qualifications
Demonstrated competency in patient care standards and infusion skill sets; advanced certification and training as applicable.
Required licensure to operate a motor vehicle with proof of liability insurance.
Basic knowledge of computer operating systems and applications, ability to apply nursing technology tools for communication and documentation.
Physical Demand Requirements
Ability to lift up to 50 pounds (maximum 75 pounds); exert up to 20 pounds of force occasionally, or 10 pounds frequently; demands include walking, standing, crouching, kneeling, turning, pivoting, balancing, stooping, reaching overhead, grasping, pushing, pulling, lifting and carrying.
Likelihood of exposure to blood, body fluids and potentially infectious materials; possible hazardous substances and environmental risks such as needles, chemicals, fumes, weather elements, animals, secondhand smoke.
Safety requirements: closed‑toe shoes, facemask, goggles, gown or gloves and a functioning communication device for routine and emergency situations.
Travel Requirements
Willing to travel 100% of the time for business purposes.
Preferred Qualifications & Interests
Bachelor of Science in Nursing from an accredited program.
One year of previous experience as an RN in a home health or acute care setting.
Salary and Benefits
Pay Range: $38.84 – $64.72 per hour.
