Ambulatory Licensed Practical Nurse - Touchpoint Senior Living
Join Community Health Network in this opportunity to support daytime care for residents at Touchpoint Senior Living.
Location 9894 E. 121st Street, Fishers, IN
Responsibilities
Float to care facilities in the Indianapolis‑area assisting the provider with the plan of care and organizing daily visits to patients with changes in acuity.
Assist with indirect patient care ordering, transcription, documentation, and help reinforce patient and family education.
Collaborate with other team members to perform other unit services as directed.
Qualifications
Graduate of a recognized accredited school of practical nursing, or graduate from a non‑accredited school of practical nursing with three years related professional nursing experience.
Licensed as a Licensed Practical Nurse (LPN) by the Indiana Professional Licensing Agency.
