Physical Therapist, PT – Home Health (Optum, part of LHC Group) $5,000 Sign‑On Bonus for External Candidates Optum and LHC Group are leading post‑acute care partners dedicated to helping people feel their best. As a Physical Therapist in Home Health, you will assess patient functional status, develop treatment plans, perform interventions to improve mobility and pain, and monitor progress. Primary Responsibilities

Evaluate patient functional status and physical therapy needs; assist the physician in developing the therapy plan of care. Provide services within scope of practice as defined by state laws, in accordance with the plan of care, and in coordination with other healthcare team members. Observe and report patient response to treatment and any changes to the supervising nurse and/or physician. Treat patients to relieve pain, restore function, and maintain maximum functioning using accepted standards of practice. Instruct patients, family/caregivers, and team members regarding the plan of care, functional limitations, and progress toward goals. Visit patients per plan of treatment, complete a progress note for each visit, and submit notes per LHC policy. Complete all patient evaluations and develop the PT plan of care within state‑specific guidelines. Report outcomes of evaluation, goals, and anticipated projected frequency of care. Required Qualifications

Current Physical Therapy licensure in state of practice. Current CPR certification. Valid driver’s license and vehicle insurance, or reliable vehicle/transportation. Ability to function in any home situation regardless of client’s age, race, creed, color, sex, disability, or financial condition. Pay is based on local labor markets, education, experience, and certifications. Hourly pay ranges from $34.23 to $61.15 per hour for full‑time employment.
