Monday–Friday, 8:00 AM – 5:00 PM (occasional outreach events after 5:00 PM)

Base Pay Range:
$24.00/hr - $26.00/hr
Position Overview The Community Health Worker (CHW) will support community members by conducting outreach, providing education on health programs and resources, and assisting clients with navigation of medical systems and coverage options. This role involves direct client engagement, collaboration with medical providers and partner agencies, and accurate documentation in state and electronic medical record systems.
Key Responsibilities
Conduct assigned community outreach activities
Represent programs at community events (e.g., outreach tables)
Deliver presentations to small and large groups
Explain program resources, eligibility criteria, and enrollment processes
Communicate and coordinate with medical providers and partner agencies
Conduct phone outreach with clients and families regarding available resources
Manage an assigned client caseload
Conduct client interviews (preferred in person; single-interview process)
Document client encounters and progress notes in EMR systems
Perform data entry related to provider communications
Maintain accurate case records in state database systems
Ensure confidentiality and compliance with all documentation requirements
Required Experience & Skills
Bilingual in English and Spanish (required)
Case management and/or care coordination experience
Knowledge of medical terminology
Understanding of medical systems and insurance programs
Experience evaluating client coverage and identifying gaps
Comfortable using Microsoft Excel
