Job Overview Entry level Registered Nurse in the Clinical Advancement Program for Home Therapy and Chronic Kidney Disease care.
Responsibilities
Perform essential functions under guidance of Program Manager, Educator, Preceptor or Home Therapy CAP RN 2-5 during first 6 months.
Complete requirements to advance to next CAP level within 24 months of hire.
Review dialysis data for Home Therapy patients, document analytical review, adjust treatment plan, notify Supervisor or Physician as needed.
Recognize patient status variations, report or collaborate with health team members.
Assess assigned patient care needs, collaborate with direct and ancillary staff.
Direct and provide safe delivery of chronic dialysis therapy to assigned patients.
Administer medications as prescribed or per algorithm, document justification and effectiveness.
Ensure correct laboratory collection, processing, shipping procedures and reschedule missed or insufficient collections.
Ensure physician orders entered and acknowledged in Medical Record.
Initiate or assist with emergency response measures.
Delegate appropriate tasks to direct patient care staff including RNs, LVN/LPNs, Patient Care Technicians.
Monitor patient care staff for adherence to policy and procedures.
Promote infection control, equipment and environmental safety.
Assess, collaborate and document patient/family basic learning needs for initial and ongoing education.
Assist in identification, evaluation, selection and education of Home Dialysis candidates and Home Partners.
Perform assessment and identify barriers of Home Dialysis candidate’s home environment and partner/family readiness to perform dialysis treatments at home.
Admit new patients to Home Therapy per policy and procedure.
Train Home Dialysis patients and Home Partners on safe and effective operation and maintenance of all Home Dialysis equipment and supplies through formal training program.
Assess and manage patients’ response to home dialysis training and therapy by following prescribed protocols. Communicate patient related issues to physician as needed.
Identify expected outcomes, document and update nursing assessment and plan of care for assigned patients through collaboration with Interdisciplinary Team.
Ensure patient awareness related to transplant and treatment modality options.
Make referrals to Social Worker and Registered Dietitian as appropriate.
Check availability of adequate emergency equipment and supplies as assigned.
Assist with medication tracking and inventory as assigned.
Serve as resource for health care team, participate in staff training and orientation of new staff as assigned.
Participate in all required staff meetings as scheduled.
Support and promote Fresenius Kidney Care mission, vision, values and customer service philosophy.
Perform all other duties as assigned by Supervisor.
Physical Demands
Direct patient care that involves heavy lifting and moving patients, frequent prolonged standing, ability to bend over, move machines and equipment up to 200 lbs, lift materials up to 30 lbs as high as 5 feet.
Work environment characteristic of health care facility with air temperature control, moderate noise levels, exposure to infectious and contagious diseases/materials.
Rotates coverage with other licensed home therapy staff as assigned to ensure reliable and adequate coverage.
Position requires participation in on-call rotation (night, weekend or as defined by individual program needs).
May be asked to provide essential functions of this position, travel to home dialysis patient homes and other locations with the same physical demands and working conditions.
Supervision
Assign oversight of Patient Care Technicians/LPNs as Team Leader or designated Nurse in charge after successful completion of all FKC education and training requirements for new employees.
Minimum 12 months experience as RN plus successful 3 months experience as RN in home peritoneal dialysis and/or home hemodialysis.
Education
Graduate of an accredited School of Nursing.
Current appropriate state licensure.
Meet the practice requirements in the state in which employed.
Experience & Required Skills
Entry level for RNs with less than 2 years of nephrology nursing experience within last two years or RNs with BSN and less than 1 year of nephrology nursing experience within last two years.
Minimum 9 months experience as a Registered Nurse (preferred).
Chronic Home dialysis or hemodialysis experience (preferred).
Successful completion of FKC Education and Training requirements for new employees.
Successful completion and maintenance of CPR BLS Certification.
Meet all conditions of employment including Ishihara's Color Blindness Test.
Successful completion of all annual required education/training/competencies.
Successful completion of ongoing training and education as assigned or required.
