Job Summary:

The VAD/Heart Transplant Coordinator will coordinate care for all patients requiring implantation of a cardiac assist device, including patients awaiting heart transplant and those with advanced heart failure. The coordinator will serve as the primary liaison with transplant cardiologists, cardiac transplant surgeons, and other physicians, and will provide patient and family education, rounds, discharge planning, and on‑call coverage. The coordinator will also conduct outreach to promote the program and maintain professional education.

Responsibilities:

Coordinate pre‑ and post‑transplant patient care and day‑to‑day clinical operations for VAD and heart transplant patients.

Liaise with transplant cardiologists, cardiac surgeons, and multidisciplinary care teams to provide comprehensive patient management.

Provide patient and family education, daily rounds, and discharge planning for VAD and transplant patients.

Cover on‑call shifts and maintain availability for transplant patient emergencies.

Conduct outreach and activity to promote the VAD/Heart Failure program and professional education.

Maintain documentation and compliance with the LVAD Practice Guidelines and ANA Nursing Scope & Standards of Practice.

Qualifications:

Minimum 3 years acute‑care nursing experience; experience in cardiology, heart failure, VAD care is preferred.

Associate’s degree in Nursing with a minimum of 10 years of relevant experience or Bachelor’s degree in Nursing with a minimum of 3 years of relevant experience.

Wisconsin RN license or a multi‑state Nursing License under the Nurse Licensure Compact.

Certified Clinical Transplant Coordinator (CCTC) or Certified Clinical Transplant Nurse (CCTN) or Acute/Critical Care Nurse (CCRN) certification is preferred.

Strong analytical, detail‑orientation, and problem‑solving skills.
