Job Title: Inspector V

Job Description Inspector V conducts on‑site inspections and investigations in long‑term care facilities, including licensed‑only nursing facilities, assisted living facilities, day activity and health services, pediatric extended care centers, and more. The inspector investigates abuse, neglect, and exploitation of adults and children with mental or developmental disabilities or physical disabilities, tests findings in hearings and court proceedings, writes detailed reports, and recommends enforcement actions.

Essential Job Functions

Conduct on‑site licensure inspections, complaint and incident investigations, and annual comprehensive surveys (25%).

Prepare written reports detailing deficiencies and providing narratives supporting regulatory findings. Summarize facts, ensure clear documentation, and recommend corrective action. (25%)

Gather evidence, analyze it, and determine if abuse or neglect occurred, following chain of custody. (25%)

Review and approve corrective action plans, inform providers of required remedial actions, recommend enforcement actions, and ensure penalties are assessed as required. (10%)

Participate in meetings, testify at hearings and court proceedings, train new hires, and perform on‑job training (10%).

Provide on‑call and emergency support, including weather events or high‑priority situations, and perform other duties assigned. (5%)

Knowledge, Skills, And Abilities

Interpret local, state, and federal laws and regulations relevant to long‑term care.

Apply professional standards of care and conduct inspections and investigations.

Establish and maintain effective working relationships.

Communicate verbally and in writing.

Work independently and prepare reports, analyze data, and exercise professional judgment.

Manage multiple priorities and work in potentially emotionally charged environments.

Operate computers, tablets, laptops, smartphones, and Office software.

Conduct interviews in various locations, including private residences and state‑operated facilities.

Work in environments where violent or dangerous individuals may be present.

Oversee the work of others.

Registrations, Licensure Requirements, & Certifications

Valid Class C (or higher) Texas driver’s license or equivalent license from another state.

Initial Screening Criteria

Experience interacting with or communicating with members of the public, in person or by phone.

Four‑year degree from an accredited college or university (preferred).

Preferred Experience

Conducting investigations.

Working with individuals with mental illness, intellectual or developmental disabilities, or physical disabilities.

Other Requirements & Additional Information

Work schedule requires flexibility, including evenings, weekends, and holidays.

Up to 80% travel statewide, overnight and weekend travel.

On‑call duties for emergencies or high‑priority situations.

Must have reliable transportation.

All applicants must pass a fingerprint criminal background check.

Active or former nursing facility administrators, certified nurse aides, or medication aides must complete licensing and misconduct checks before hiring.

Hiring imposes conflict of interest disclosure and non‑employment at HHSC‑regulated facilities upon acceptance.
