is seeking a dedicated Home Health Physical Therapist to help patients regain strength, mobility, and independence from the comfort of their homes. The role requires strong clinical skills, excellent communication, and a collaborative approach with the care team.
Pay & Benefits
Base pay range: $85,400 – $117,500 per year
Sign‑on bonus: $10,000*
Pay per visit/unit: $54.00 – $76.00
Full‑time, 40 hours per week (weekly hours: 40)

Responsibilities
Plan and administer skilled physical therapy treatment and training for patients with injuries, illnesses, and functional disabilities.
Screen and test patients’ physical strengths to support physician assessment and develop measurable training objectives.
Develop and implement rehabilitation programs consistent with the physician’s plan and overall patient goals, adjusting treatment as needed.
Collaborate with physicians and clinical team members to obtain additional patient information and revise therapy programs.
Provide written care plans and supervise Physical Therapy Assistants and Home Health Aide staff, evaluating staff performance and documentation completion.
Document patients’ care observations, interventions, and evaluations accurately and promptly, including physician orders requiring signature.
Report patient progress to physicians, Clinical Manager, staff, patient, and family; submit evaluation, treatment plans, progress reports, and discharge summaries to the supervisor and care management staff.
Qualifications
Degree from an accredited Physical Therapy Program approved by the APTA.
Current and unrestricted Physical Therapy license.
Current CPR certification.
Minimum one year physical therapy experience preferred.
Strong organizational and communication skills.
