Job Summary Folkston Park Care & Rehab is in search of a dedicated and compassionate Physical Therapist Assistant (PTA) to join our IN-HOUSE team in a long-term care (LTC) facility! Under the supervision of a licensed Physical Therapist, the PTA will assist in providing physical therapy services to residents in order to enhance their quality of life, restore mobility, and promote functional independence.

Key Responsibilities

Implement physical therapy treatment plans developed by the Physical Therapist in accordance with physician orders and facility protocols.

Provide therapeutic interventions such as exercises, gait training, balance activities, and use of assistive devices.

Monitor patient progress and report any changes or concerns to the supervising Physical Therapist.

Assist residents with mobility and physical functioning to support daily living activities.

Educate residents, caregivers, and staff on mobility techniques and use of adaptive equipment.

Maintain accurate and timely documentation of treatments, progress notes, and other required records.

Ensure a safe, clean, and supportive environment for all residents.

Collaborate with interdisciplinary team members including nurses, occupational therapists, and speech-language pathologists to support resident care plans.

Adhere to all federal, state, and facility regulations and policies.

Qualifications

Associate degree from an accredited Physical Therapist Assistant program.

Current state licensure or certification as a Physical Therapist Assistant.

Prior experience in a long-term care, skilled nursing, or geriatric setting preferred.

Strong communication, organization, and interpersonal skills.

Ability to work effectively as part of a team and independently with minimal supervision.

Working Conditions

Primarily in a skilled nursing or long-term care facility setting.

Physical demands include lifting, standing, bending, and assisting with patient mobility.
