Home Health RN Registered Nurse (PRN) – Empath Home Health - South Pinellas county
Empath Home Health, a member of Empath Health, is seeking a compassionate
PRN Home Health RN
to provide skilled nursing care for patients in their homes throughout
south Pinellas county
(St. Petersburg, Clearwater, Largo, Pinellas Park, Seminole, Gulfport, and surrounding areas). In this PRN role, you’ll deliver individualized care, build trusted patient relationships, and help people manage their health with dignity and independence.
Locations:
Home health visits in
south Pinellas county
(St. Petersburg, Clearwater, Largo, Pinellas Park, Seminole, Gulfport, and surrounding areas)
Schedule:
PRN role offering 5-10+ visits per week. Required weekend/holiday on-call rotations.
With more than 60 years of expertise, Empath’s Home Health service line provides skilled nursing, therapy, and personal care services that help people recover and maintain independence at home. Part of Empath Health’s not-for-profit network, the program serves patients across 16 Florida counties and is accredited by the Joint Commission and the Accreditation Commission for Health Care.

What You’ll Do
Perform comprehensive patient assessments, including OASIS assessments at required intervals
Develop, initiate, and revise individualized plans of care as patient needs evolve
Deliver skilled nursing services and treatments as outlined in the plan of care
Counsel and educate patients and families on disease processes, self-care techniques, and prevention strategies
Collaborate with the interdisciplinary team to ensure seamless, holistic care

What You’ll Need
Active Florida RN license
Prefers BSN or MSN degree
Minimum of 1 year of direct patient care experience; prior home health RN experience strongly preferred
Strong assessment, critical thinking, and patient education skills
Willingness to participate in weekend and holiday on-call rotations
