Per Diem Supervising Physical Therapist

to oversee care provided by PTAs in the Center Moriches area.

The PT's primary responsibility will be to perform initial evaluations, progress notes, and required supervisory visits for PTAs. This is a high-need area with an immediate need for PTA supervision coverage. There is also the option to take on ongoing treatment visits in addition to the supervisory role, depending on your availability and interest. This is a flexible per-diem position, with competitive per-visit rates for both evaluations and supervisory visits. Location:

options to fit your preferred work style Opportunities for professional growth and development A professional, supportive and fun work environment fostered by a team of well-respected clinicians who take pride in the challenges of meaningful work Focus on Patient Care - Quick EMR (No Oasis required) Flexibility and Autonomy - see patients any 7 days of the week Preference given to see patients in your primary geographic area Responsibilities: Accept and treat patients within your designated geographic area Perform initial evaluations, progress notes, and required supervisory visits for PTAs Evaluate patients’ needs for physical therapy in the home setting and develop individualized treatment plans Develop a plan of care and treatment goals in consultation with the physician, patient, and family Plan, prepare and implement customized therapy programs to restore function and promote independence Instruct family members, PTAs, Home Health Aides, and caregivers on appropriate therapy techniques Provide timely and accurate documentation for each visit Communicate effectively with patients, families, and interdisciplinary team members, especially around discharge planning Deliver care in compliance with agency policies and professional practice standards Qualifications

Physical Therapist Program Graduate Licensed Physical Therapist in the state of New York or eligible. Mandatory Certifications: First Aid, AED, & CPR Certifications. Home health experience is preferred but not required - We will provide you with a mentor to help with documentation and compliance. Ability to travel to patients’ homes within the assigned geographic territory
