Targeted Case Manager - Child and Family
Location: Waynesville, NC
Job Summary Targeted Case Manager serves individuals with mental health and substance abuse disorders who require assistance to gain access to necessary care: medical, behavioral, social, and other services appropriate to their needs. Services include case management assessment, person‑centered planning, referral and linkage, monitoring, and follow‑up. The Targeted Case Manager will follow specific policies and procedures for the Family Safety Program and Family Intensive Treatment.
Essential Duties And Responsibilities
Provide case management functions: assessment, person‑centered planning, referral and linkage, and monitoring and follow‑up.
Document all treatment and clinically significant events in the Electronic Health Record as required by CIHA policies and quality assurance standards.
Fulfill duties as outlined in CIHA policy and procedure for the specific population served (adults, children and families, Buprenorphine Clinic).
Gather and document necessary information across medical, functional, psychosocial, financial, and cultural domains.
Develop person‑centered treatment plans to achieve clinical outcomes.
Provide timely information to treatment team members regarding client status, progress, and safety.
Use judgment for risk assessment and crisis intervention per CIHA policy.
Collaborate with medical staff and other health professionals/agencies to facilitate patient care, discharge, and follow‑up.
Consult with patients and families on financial, environmental, and legal matters relevant to treatment.
Implement discharge plans and coordinate transfers to other behavioral health facilities.
Implement privacy and confidentiality policies, including HIPAA and CIHA standards, for patient record management.
Attend clinical staffing sessions and contribute to treatment plan adjustments.
Participate in program improvement meetings, recommending corrective actions and policy changes.
Provide written, telephone, and face‑to‑face consultation services to schools, courts, probation officers, hospitals, and community agencies.
Represent CIHA at progress review and emergency meetings involving high‑risk situations.
Participate in community planning and program implementation for service coordination.
Collect and report data necessary for client care per procedure and supervisor direction.
Pursue continuing education through in‑service training and approved workshops.
Meet productivity standards in billable/reimbursable case management services when required.
Utilize electronic medical record and associated software within the CIHA system.
Support CIHA primary care nurse care managers as needed.
Perform additional duties as assigned.
Education/Experience/Minimum Qualifications
Individual who holds a license, provisional license, certificate, registration, or permit issued by the governing board regulating a human service profession, or registered nurse licensed in North Carolina with four years of full‑time experience in mental health, substance abuse, or related fields.
Graduate with a Master’s degree in a human service field and one year of full‑time experience in mental health or substance abuse with the population served, or substance abuse professional with one year supervised experience in counseling.
Graduate with a Bachelor’s degree in a human service field and two years of full‑time experience in mental health or substance abuse, or substance abuse professional with two years supervised experience.
Graduate with a Bachelor’s degree in a field other than human services and four years of full‑time experience in mental health or substance abuse, or substance abuse professional with four years supervised experience.
Substance Abuse Professional, Certified Alcohol and Drug Counselor (CDAC), Certified Clinical Supervisor (CCS), or Licensed Clinical Addiction Specialist (LCAS) may qualify.

Skill in establishing rapport and assessing psychological, behavioral, and psychosocial aspects of patient problems.
Ability to maintain effective working relationships with caseload members, families, professionals, and community resources.

Understanding of the impact of mental illness on treatment negotiation and recommended approaches.
Willingness to learn more about Cherokee culture and population considerations.
Complexity of Duties The position requires maximizing resources to develop and maintain behavioral health services for EBCI children and families. It demands decision making, problem solving, and often delivering care when individuals are highly agitated or disconnected from reality.
Supervision Received Works under the direct guidance of the Family Safety Supervisor, Child Services Manager, and/or Behavioral Health Clinical Director. Works independently within licensing regulations where discretion is granted to achieve departmental goals.
Responsibility for Accuracy Accuracy in documentation and data interpretation is critical to client care and resource allocation. Inappropriate data handling may result in liability for CIHA or serious health risks to patients.
Contact With Others Interacts frequently with coworkers, hospital staff, patients, and health professionals. May serve on committees, boards, or advisory councils. Professional conduct, courtesy, and confidentiality are required.
Confidential Data Maintains strict confidentiality of patient information in accordance with EBCI policy, CIHA confidentiality policy, HIPAA, and other federal requirements. Violations may result in disciplinary action.
Emergency Privileges In emergencies, qualified Medical Staff may perform actions necessary to save a patient’s life or prevent serious harm as permitted by license.
Mental/Visual/Physical Environment Job duties can be mentally and emotionally stressful. Physical activities require mobility, sitting, reaching, bending, and sometimes traveling. Some physical exertion may be required. Must be able to lift 30 lbs.
Work Environment Primarily at the PHHS building with occasional community, hospital, inpatient, or outpatient work. Moderate risks of infectious disease, radiation, electrical hazards, and chemical exposure. OSHA guidelines must be followed. Some travel, including occasional overnight travel, may be required.
