– Community Based Outpatient Clinic

Job Summary The primary purpose of this position is to serve as the Registered Nurse Care Manager, who represents the interests of veteran patients in all areas of the clinic and coordinates care with VA medical Center and stakeholders.

Essential Functions

Provide nursing care based on physician, mid-level provider and CBOC Clinic Manager's orders to meet patients' physical, mental, and emotional needs.

Supervise support staff in delivery of care.

Lead and participate in PACT team huddles.

Work with the Primary Care Physician (PCP) to ensure adequate staffing for quality care delivery and promotion of continuity of care between the VAMC and the clinic.

Maintain universal precautions and infection control practices.

Supervise medical assistants and/or licensed practical nurses who gather initial patient information.

Prepare patients for examination and assist the PACT Provider upon request.

Provide smooth and timely flow of patients per VA access standards and triage protocol.

Demonstrate triage activities for all walk-in patients and medical phone inquiries.

Administer medications per facility protocols as prescribed by provider.

Perform CPR and assist during respiratory and cardiac arrest procedures.

Perform nursing procedures such as dressing changes, suture removal, oxygen administration, wound irrigations, catheterizations, and venipunctures.

Prepare patients for laboratory procedures by providing information and instructions.

Set up and operate therapeutic and emergency equipment, including monitor, EKG, oxygen, and portable suction.

Document all pertinent patient information in medical records and VISTA.

Maintain VISTA clinic reminder tracking system for each enrolled patient.

Assist in scheduling patients for initial and follow-up appointments per VA access standards.

Keep patients informed regarding their plan of care, including lab and diagnostic results.

Provide health materials, resources, and education to patients and families.

Assist providers in ensuring electronic encounter documentation is complete and accurate.

Participate in the Performance Improvement Program between subcontractor(s) and the VAMC.

Ensure all required reports, clinical and administrative documentation is completed accurately and timely.

Manage day-to-day CBOC operations.

Comply with all applicable regulations, directives, and standards.

Maintain confidentiality and patient privacy.

Perform duties of the CBOC Clinic Manager when absent (if applicable).

Perform other work-related duties as assigned.

Required Experience and Skills

Three years of experience in Medical Surgical Unit, Outpatient Facility, Primary Care and/or Adult Health (preferred).

Current, unrestricted nursing license in Idaho.

Current CPR/BLS certification from AHA or MTN.

Knowledge of professional nursing principles, procedures, and techniques.

Ability to assess patient condition, recognize adverse signs, and react swiftly in emergencies.

Ability to set realistic education goals for patients.

Ability to develop and provide individualized and group counseling, guidance, and health instruction.

Ability to teach patients/families proper home health care activities.

Knowledge of pharmacological agents used in patient treatment.

Demonstrated customer service skills with excellent verbal, written, and telephone communication.

Knowledge of Microsoft Office, Electronic Medical Records, and computer proficiency.

Detail-oriented and capable of multitasking.

Required Education

RN graduate from a school of professional nursing approved by the appropriate state-accrediting agency.

American Nursing Accrediting Body (ACEN or CCNE) accredited program (at time of completion).

Bachelor of Science in Nursing (BSN) preferred.

Working Conditions / Physical Requirements

Moderate physical activity, handling objects up to 25 pounds and standing/walking for more than four hours per day.

Typical interior/office work environment.

Must respond to medical emergencies requiring fast walk or jog within clinic.

Pay Starting at $31 per hour.
