Perform in‑home PT evaluations and create personalized care plans

Collaborate with a multidisciplinary care team
Document care using mobile‑friendly EMR
Requirements:
Active MA PT license
DPT or MPT from an accredited program
CPR certified
1+ year of experience preferred (new grads welcome)
Reliable vehicle & valid driver’s license
