Behavioral Hlth Licensed Practical Nurse
Center for Human Development (CHD) is hiring a Behavioral Health Licensed Practical Nurse (LPN) to join the team at Heron House, an Enhanced Medical Group Living Environment (EMGLE) located in East Longmeadow, MA.

Behavioral Health Licensed Practical Nurse
Employment Type: Full‑time, on‑site
Location: East Longmeadow, MA
Job Overview As an LPN, you will work under the supervision of a charge nurse or RN and collaborate with a team of LPNs, CNAs, and Direct Care staff to provide quality care and support to residents.
The facility is a brand‑new 18‑bed EMGLE with a small LPN‑to‑client ratio, offering around‑the‑clock nursing and personal care assistance to persons with complex mental illness and chronic medical conditions.
Responsibilities
Medication administration and management; assist in medication audit processes.
Use CHD’s Safety Care training skills to deescalate and redirect participants during crises.
Attend participant care plan meetings and resource meetings.
Provide direct care, including personal hygiene assistance and help with bathroom travel.
Deliver activities of daily living: assist with meals, feeding, ambulation.
Administer non‑sterile dressings, surgical preps, ice packs, and heat treatments.
Monitor and record vital signs, weight, intake, and output; maintain stability of residents.
Offer adjunct care as needed.
Qualifications
Licensed practical nurse in the State of Massachusetts.
At least 1 year of experience as a LPN.
Preferred: One year experience in a behavioral health setting; working with the elderly, people with disabilities, or those with behavioral challenges and cognitive disabilities.
Preferred: Completed Safety Care Training, which includes physical restraints, de‑escalation, and redirection techniques (training provided).
Valid driver’s license.
Physical ability to lift 45 lbs or more, stock supplies, and walk or stand for extended periods.
Commitment to completing assigned training requirements in compliance with CHD and the Massachusetts Department of Mental Health.
Benefits Competitive hourly wage of $38.50 per hour, with additional holiday bonus pay for New Year’s Eve, Fourth of July, Labor Day, Memorial Day, and Christmas Day. Full benefit package available. Flexible shift schedule, all shifts available, with requirement of working every other weekend.
