The Resident Advocate / Care Manager I advocates for residents and provides intensive care and intensive case management services to eligible clients, meeting participants at least weekly. The Resident Advocate / Care Manager is the primary point of contact to coordinate services and care for assigned clients. Each Resident Advocate / Care Manager handles a caseload of individuals in need of a wide variety of human services resources and provides the full range of services available, as described below: Intensive Case Management Duties include: Duties And Responsibilities
Conduct comprehensive screenings and assessments with individuals (or families) to collect functional, environmental, psycho‑social, financial, employment, housing, educational, and health information, as appropriate Assess barriers facing the clients and develop a case plan Coordinate individualized planning with clients to meet short‑ and long‑term needs Provide service linkages and support systems to ensure identified needs are met Coordinate case conferences with residents, consult with service team, monitor progress of clients on case plan, assist clients to meet the agreed‑upon goals Act as advocate for client before judicial, community, social service, and administrative bodies as needed; facilitate support groups and client participation as appropriate Work with Community Property Management to provide service linkages and support systems to residents to assure individual needs are met Maintain daily contact to identify new and ongoing service needs Coordinate access to supportive services including, but not limited to, health services, medical appointments, and crisis intervention, as needed Other duties as required Essential Duties
Assessment: through individual meetings on a regular basis and as part of Intensive Case Management team develop a detailed plan of action to meet the clients needs and find the necessary resources to meet client goals Coaching and Social Support: accompany participants to activities and services Coordinate Case Management as part of team, connecting participants to appropriate services Advocacy for participants before other agencies/community settings Capacity Building: motivating participant progress Coordinate the participant’s care in collaboration with the participant, and all members of the participant’s care team Other duties as required and may be subject to change Qualifications
Must be able to pass a fingerprint clearance, background check, including criminal history, personal references, employment and education verifications Education
Bachelor’s degree in Social Work, Mental Health or other related discipline OR Two (2) years of experience providing direct mental health services or ICMS Experience
Minimum one (1) year of experience working with homeless individuals Have experience working with participants with mental illness, chronic health issues, and substance use disorders Preferred Qualifications
