Position Title Case Manager
Corps/Department Salvation Army, McKinnell House
Reports To Program Manager
FLSA Status Full-Time; Non-Exempt
Scope of Position The Case Manager provides direct case management for up to 16 families who reside at McKinnell House. The position performs client assessments, assists with client goal planning, and reviews progress toward the goal to facilitate families transitioning from homelessness to independent living. The Case Manager also acts as an advocate of the client families as they apply and seek services within the community.
Minimum Qualifications
Bachelor’s degree in social work or related field or two years of case management experience.
Proficient in the use of general office equipment including computers.
Knowledge and understanding of client and organization confidentiality requirements.
Demonstrated ability to manage time effectively and to work harmoniously with clients, visitors, officers, and other personnel.
Currently trained in CPR and first aid certificates or able to obtain within 60 days of employment.
Current driver’s license with acceptable driving and criminal history records.
Essential Functions
Conduct preliminary intake of all potential residents, match potential families with room availability, and enter data into appropriate databases such as HMIS, Bridges, etc.
Complete intake paperwork, obtaining information regarding school for children, current medications, identification, and background history, identify any State Children Services issues, and provide appropriate referrals when necessary.
Issue Resident Handbook and go over Case Management expectations.
Provide direct case management services for shelter residents as needed, and meet with residents weekly to do goal planning and goal reviews and determine progress.
Conduct client assessments to determine strengths and difficulties preliminary to case planning and communicate areas of concern or that need special attention (i.e., special diets, disabilities).
Address challenges and difficulties and help residents find solutions relating to employment, income, housing, education, and childcare.
Advocate for homeless clients by attending appropriate community networking and task force meetings, and assist residents in completing applications, providing community resource information, and providing referrals and problem‑solving assistance.
Complete accurate and thorough documentation of contact with residents, other social service agencies, and programs as soon as possible following contact; assist with planning for successful discharge of residents.
Communicate with the CMS regarding the progress and challenges of residents and submit monthly activity reports to CMS in a timely manner.
Advocate for homeless clients by attending appropriate community meetings, participate in task forces and network with other agencies and programs.
Provide training to students and volunteers as requested; model team behavior; attend staff meetings and gatherings.
Assist in crisis intervention by obtaining police, medical, psychiatric or other emergency services assistance as appropriate.
Perform other duties as assigned.
Work Environment Work is performed in a heated and ventilated institution setting. Noise level is low. Exposure to extreme temperatures is not a requirement of this position. The use of personal protective equipment may be required in accordance with the Hazard Communication Plan.
Physical Demands
Ability to speak, sit, walk, stand, bend, climb, kneel, and twist on an intermittent basis; grasp, reach, handle, finger, push or pull objects.
Hearing and vision acuity at or corrected to within normal levels.
Ability to lift up to 30 lbs., or more with assistance.
Ability to operate a motor vehicle in daylight, darkness, or in inclement weather conditions.
